چکیده
اهداف: هدف این پژوهش، طراحی و ارزیابی مداخلهای شناختی–رفتاری–هیجانی و اجتماعی برای بیماران قلبی دارای فشارخون بالا و اختلال نعوظ بود. این مداخله با تمرکز بر بهبود عملکرد نعوظ، رضایت جنسی، ارتباط بینفردی و سلامت روان از طریق آموزش جنسی، حمایت روانی، راهبردهای ارتباطی و اصلاح سبک زندگی اجرا شد.
روشها: ۳۰ مرد ۴۵ تا ۶۰ ساله مبتلا به فشار خون بالا و اختلال نعوظ بهطور تصادفی به دو گروه تقسیم شدند: گروه آزمایشی که مداخله ۱۲ جلسهای را دریافت کرد و گروه کنترل که تحت مراقبت معمول باقی ماند. برنامه شامل آرامسازی، مصاحبه انگیزشی، بازسازی شناختی (بر اساس مدل متز و همکاران)، آموزش روانجنسی و مهارتهای صمیمیت بود. بر تغییر سبک زندگی و کاهش BMI نیز تأکید شد. ابزارهای سنجش شامل پرسشنامه IIEF و مقیاس ارتباط جنسی (SCS) بود.
یافتهها: گروه آزمایشی بهبودهای معنادار و پایداری را نشان داد. نتایج آنووا با اتدازه گیری مکرر نشاندهنده اثر قوی زمان × گروه در عملکرد نعوظ و اضطراب ارتباط جنسی بود (p < 0.001). کاهش معناداری در BMI ثبت شد (χ² = 22.842، p = 0.000) و تفاوت بینگروهی در پیگیری نیز معنادار بود (U = 52.000، p = 0.012). در پیگیری، عملکرد نعوظ، میل و رضایت جنسی، رضایت از رابطه، و کاهش اضطراب ارتباط جنسی در گروه آزمایشی بهمراتب بهتر از گروه کنترل بود.
نتیجهگیری: این مداخله یکپارچه با رویکرد روانشناختی و اجتماعی، همراه با اصلاح سبک زندگی، توانست عملکرد جنسی و کیفیت روابط را در مردان مبتلا به فشار خون بالا و اختلال نعوظ بهبود بخشد.
