Abstract

Objective: The aim of this study was to compare the effectiveness of emotion-focused therapy and cognitivebehavioral therapy on pain coping strategies and pain catastrophizing in patients with pain disorder.
Method: The research method was a quasi-experimental with pretest-posttest design and a control group and a twomonth follow-up. The statistical population of the study included all women with psychosomatic problems and pain
who referred to Manoushan and Behsa counseling centers in Tehran. The sample size included 60 patients (three groups
of 20). The research instruments included the Rosenstileand Keefe (1983) Pain Coping Strategies Questionnaire and
the Bishop and Pivik (1995) Pain CatastropheScale. The emotion-focused experimental group received 12 training
sessions and the cognitive-behavioral experimental group received 10 sessions training, but the control group did not
receive any intervention during the study.
Results: The results of mixed variance analysis and Benferroni pairwise comparison test showed that both emotionfocused therapy and cognitive-behavioral therapy have significant impact on the components of pain reinterpretation,
pain catastrophizing, behavioral activity, and coping efficiency (P <0.05), but they do not have significant effect on
the components of return attention, ignoring pain, prayer and hope, and self-talking (P <0.05). The results also showed
that both treatments have significant effect on pain catastrophizing (P<0.05). Also, no significant difference was
observed between the two experimental groups (P <0.05).
Conclusion: Therefore, the results of the present study show that emotion-focused therapy, like cognitive-behavioral
therapy, can be used for patients with pain disorders and improve their cognitive factors.

Abstract [Persian]

هدف پژوهش حاضر مقایسه اثربخشی درمان هیجان‌مدار و درمان شناختی رفتاری بر راهبردهای مقابله با درد و فاجعه‌سازی درد در بیماران مبتلا به اختلال درد بود. روش پژوهش حاضر نیمه آزمایشی از نوع طرح پیش آزمون - پس آزمون با گروه کنترل و پیگیری دو ماهه بود. جامعه آماری پژوهش شامل کلیه زنان مراجعه کنندگان به مرکز مشاوره مانوشان و بهسا شهر تهران با مشکلات و دردهای روان تنی بود. حجم نمونه شامل 60 نفر از این بیماران بود (سه گروه 20 نفری). ابزار پژوهش شامل پرسشنامۀ راهبردهای مقابله با درد روزنشتایل و کیف (1983) و مقیاس فاجعه‌سازی درد بیشاپ و پیویک (1995) بود. برای گروه آزمایش هیجان مدار 12 جلسه و برای گروه آزمایش شناختی رفتاری 10 جلسه مداخله اجرا شد. ولی گروه کنترل مداخله ای در طول پژوهش دریافت نکرد.نتایج تحلیل واریانس آمیخته و آزمون مقایسه زوجی بن‌فرونی نشان داد که دو درمان هیجان مدار و درمان شناختی – رفتاری بر مولفه های تفسیر مجدد درد، فاجعه آفرینی درد، فعالیت رفتاری و کارآمدی مقابله اثربخشی معناداری دارند (05/0>P)، ولی بر مولفه های توجه برگردانی، نادیده انگاشتن درد، دعا و امیدواری و گفتگو با خود اثربخشی معناداری ندارند (05/0<P). همچنین نتایج نشان داد که هر دو درمان بر فاجعه‌سازی درد تاثیر معناداری دارند (05/0>P). همچنین تفاوت معناداری میان دو گروه آزمایش مشاهده نشد(05/0<P). بنابراین نتیجه پژوهش حاضر نشان می‌دهد که درمان هیجان‌مدار مانند درمان شناختی رفتاری می‌تواند برای بیماران با اختلال درد و بهبود عوامل شناختی آن مورد استفاده قرار گیرد.
