Abstract

Objective: This study compared health-related quality of life (HRQoL) and stress appraisal among overweight, obese, and post-bariatric surgery women.
Method: We used a descriptive, causal-comparative design with 90 women. Each group—overweight, obese, and post-bariatric surgery—contained 30 participants. Participants were purposively sampled from Imam Reza Hospital. The Short Form Health Survey (SF-36) assessed physical (PCS) and mental (MCS) HRQoL. The Stress Appraisal Measure (SAM) evaluated perceived stress. We analyzed data using MANOVA and Tukey’s post hoc tests in SPSS 26.
Results: MANOVA showed significant group differences (F = 8.65, p < 0.001, η² = 0.23). Tukey’s post hoc tests found that overweight women had higher PCS and MCS scores and lower stress appraisal than both obese and post-bariatric groups. The post-bariatric group had higher PCS and MCS than the obese group. Stress appraisal did not differ significantly between these two groups.
Conclusion: Bariatric surgery improves HRQoL compared with obesity, but not to levels seen in the overweight. High stress remains after surgery. These findings highlight the need for psychological support and early interventions to maintain HRQoL and manage stress.

هدف: این مطالعه با هدف مقایسه کیفیت زندگی مرتبط با سلامت و ارزیابی استرس در زنان دارای اضافه‌وزن، چاق و سابقه جراحی باریاتریک انجام شد.
روش‌ها: طرح پژوهش حاضر از نوع توصیفی-مقایسه‌ای بود. نمونه پژوهش شامل ۹۰ زن بود که به‌صورت هدفمند انتخاب و به‌طور مساوی در سه گروه اضافه‌وزن، چاق و دارای سابقه جراحی باریاتریک تقسیم شدند. برای ارزیابی مؤلفه‌های جسمانی (PCS) و روانی (MCS) کیفیت زندگی، از پرسشنامه کیفیت زندگی(SF-36) و برای سنجش استرس ادراک‌شده، از مقیاس ارزیابی استرس (SAM) استفاده شد. داده‌ها با استفاده از تحلیل واریانس چندمتغیره (MANOVA) و آزمون تعقیبی توکی در نرم‌افزار SPSS نسخه ۲۶ تحلیل شدند.
یافته‌ها: نتایج MANOVA تفاوت معناداری بین گروه‌ها نشان داد. (F = 8.65, p < 0.001, η² = 0.23) آزمون توکی حاکی از آن بود که زنان دارای اضافه‌وزن در مقایسه با گروه‌های چاق و دارای سابقه جراحی باریاتریک، نمرات بالاتری در PCS و MCS و سطح پایین‌تری از ارزیابی استرس داشتند. همچنین، گروه با سابقه جراحی باریاتریک نسبت به گروه چاق، نمرات بالاتری در PCS و MCS کسب کرد، اما تفاوت معناداری در ارزیابی استرس بین این دو گروه مشاهده نشد.
نتیجه‌گیری: جراحی باریاتریک در مقایسه با وضعیت چاقی، کیفیت زندگی مرتبط با سلامت را بهبود می‌بخشد، اما به سطح کیفیت زندگی مشاهده‌شده در زنان دارای اضافه‌وزن نمی‌رسد.با این‌حال، ارزیابی استرس پس از جراحی همچنان بالا باقی می‌ماند. این یافته‌ها بر اهمیت ارائه حمایت‌های روان‌شناختی جامع پس از جراحی و مداخلات زودهنگام برای زنان دارای اضافه‌وزن به‌منظور حفظ کیفیت زندگی و مدیریت مؤثر استرس تأکید می‌کنند.
